Eight states and the District of Columbia legalized marijuanafor recreational purposes, a legal shift accompanied by a striking reversal inpublic opinion: For the first time on record, a majority of Americans nowsupport legalization of the drug.   How Obamacare Is Benefiting Americans The recent Supreme Court decision upholding Obamacare ensures thatthe law will continue to expand access to important health services, improve thequality of care we receive, and control rising health care costs. To ensurethose with pre-existing conditions can access the care they need immediately, the Affordable Care Act created the Pre-existing Condition Insurance Plan, which includes coverage for primary and specialty care, hospital services, andprescription drugs. It includes more than $200 million in funding to support andexpand school-based health centers, which provide primary care, dental care, behavioral health services, and substance abuse counseling to youth nationwide. The Affordable Care Act includes $10 billion in government funding to supportefforts to expand and strengthen the primary care workforce and to improvepublic health through the Prevention and Public Health Fund.Additionally, the capacity of the National Health Services Corp aprogram of the Department of Health and Human Services that supports healthcare providers serving medically underserved communities, tripled in the lastthree years due to combined earlier investments made through the AffordableCare Act, the American Recovery and Reinvestment Act, and annualappropriations.

America Change During the Obama Years American public opinion changed in significant ways over thecourse of Barack Obamas eight years in the White House, including on issuessuch as the economy, race relations, and the level of confidence the public hasin different aspects or actors in the government. By most measures, Americansviews of the economic situation improved significantly over the eight years ofObamas presidency. In Obamas first year as president, 23% of Americans saidthey were better off financially than the year before. Americans, however, wereno more satisfied with the position of the U.

S. in the world under Obama than theyhad been in the late years of the Bush presidency. Less than four in 10Americans were satisfied with the position of the U.S. in the world in 2016,essentially equivalent to the first reading in the Obama years and marginallybetter than the 30% in 2008, Bush’s final year in office. How does the Obamacare actually work?Under the law, most people arerequired to have health insurance or pay a fine, and insurers have to take allcomers, even if they’re sick.

For people without on-the-job coverage, the lawcreated health insurance marketplaces where consumers can buy plans fromprivate insurers or sign up for Medicaid. Plans sold on the marketplaces haveto at a minimum cover 10 services that are considered essential, includinghospitalization, prescription drugs, and doctor visits, and insurers can’tcharge people higher premiums because they have medical conditions that mightmake them more expensive to insure.Affordable health careimportant in the United States? The underlying fragmentation of the health caresystem is not surprising given that health care providers do not have thepayment support or other tools they need to communicate and work togethereffectively to improve patient care. Reforming our health care delivery systemto improve the quality and value of care is essential to address escalatingcosts, poor quality, and increasing numbers of Americans without healthinsurance coverage. Chronic Disease Management, Primary Care Coordination, andHealth Information Technology There is strong evidence that particularapproaches or programs in these areas can improve quality and health outcomes. Changingprovider reimbursement Moving away from a focus on the volume and intensity ofservices provided and toward accountability for overall cost and quality isessential for supporting integrated delivery system reforms. Many valuableservices that providers already deliver, such as effective preventive care orcoordinated post hospitalization care, are generally underprovided becausedoctors and hospitals do not have adequate financial or other support toprovide them. Without payment reforms that give providers the support they needto be increasingly accountable for delivering better care at lower overallcost, individual, incremental delivery reforms or interventions are unlikely tobe adequate to address the major gaps in quality and value that currently existin the U.

S. health system. The funding cut and executive order could result inhigher insurance premiums for some Obamacare customers and lower premiums forless regulated coverage for those who want to try new insurance options. Therules governing Obamacare insurance, the subsidies to help middle-income peoplebuy it, the expansion of Medicaid to more poor adults, and Obamacare’s manyother provisions touching health and health insurance remain the law.The executive order could result in real changes for some people in theinsurance market.

But those changes are not the same thing as eliminating the health care law. Itappears that a coming rule will expand a kind of short-term insurance planintended for people who are in between jobs, making those plans available tomore consumers who wish to find an insurance option that is cheaper and lesscomprehensive than those available in the Obamacare marketplaces. If a lot ofpeople decide to buy these short-term or association insurance plans, that willmost likely leave fewer Americans in the Obamacare markets. The results are clear: Health care reform works and has alreadybenefited millions of seniors, women, children, and young adults.

If at twoyears the Affordable Care Act is already helping millions of Americans accessquality, affordable health care, we have a lot more to look forward to.
